Above and Beyond: How LIFTT Transforms Lives

Living Independently for Today and Tomorrow (LIFTT) has been a lifeline for individuals with disabilities and aging adults in southeastern and south-central Montana. At its heart, LIFTT is about breaking barriers helping people stay in their homes, connect with their communities, and live full, independent lives. Through practical support and compassionate care, LIFTT has become a trusted resource and partner for those seeking a better quality of life.

Founded to provide tools, resources, and advocacy for individuals with disabilities, LIFTT has grown into a vital organization serving 18 counties, including Yellowstone, Big Horn, and Rosebud. The organization operates under the Workforce Investment and Opportunity Act (WIOA) and adheres to its mandate to provide six core services: information and referral, individual and systems advocacy, peer support, independent living skills training, institutional transition and diversion, and youth transitions.

Freeda Mook has helped shape LIFTT’s trajectory. “I’ve fulfilled several roles at LIFTT,” she shares. “I now work in Database Management, responsible for Quality Control and Training, working closely with the Executive Director and team members. I do my best to impart my knowledge so they can be successful in their service.”

LIFTT works with individuals through Community First Choice (CFC) and Self-Directed Personal Assistance Services (SDPAS) programs. These programs provide long-term supportive care tailored to each client's needs, allowing them to stay in their homes and communities rather than move to institutional settings. Clients, called consumers, choose their personal care assistants (PCAs) and direct their care based on their unique needs.

The intake process ensures that every consumer receives personalized care. Facilitators guide them through the application process, assist with paperwork, and coordinate services such as meal preparation, mobility assistance, hygiene support, and more.

Another vital initiative at LIFTT is the Community Health Worker (CHW) program, which, as Freeda explains, “serves as a liaison between physicians, nurses, and the people they serve, enhancing the health and wellness of communities.” This program bridges the gap between medical services and individuals, ensuring everyone can access the necessary care.

LIFTT’s Personal Care Assistant Program exemplifies its commitment to empowering individuals and avoiding institutionalization. The program provides essential services to help consumers remain in their homes and communities.

Another standout initiative is the Diabetes Prevention and Management (DPM) program, which offers nutrition education, fitness programs for all abilities, and resources like a Low Glycemic Index Cookbook. These programs address many consumers' critical health challenges and equip them with the tools to lead healthier lives.



LIFTT’s impact can best be seen through the stories of individuals who have benefited from its services:

  • Neil: Living with heart and renal failure, Neil has been part of the CFC/SDPAS program since 2019. With the help of his daughter as his PCA, Neil has been able to stay in his home, maintaining his independence while receiving assistance with daily tasks like bathing, meal preparation, and mobility.
  • Cordelia: Diagnosed with Type II diabetes and several other health complications, Cordelia joined the program in 2021. She received vital support with hygiene, exercise, meal preparation, and medical escorting. While her Medicaid eligibility changed, LIFTT’s services helped her navigate her journey with dignity and independence.
  • Scott Freeland: After a life-altering traffic accident left him paraplegic, Scott joined LIFTT in 2023. He received support with tasks like dressing, mobility, and community integration, which allowed him to remain in his home and adapt to his new reality.
  • Sylvester “Champ” Ervin: At just 14 years old, Champ has been able to live at home with assistance from his caregivers and mother. Through the program, he has gained independence and has engaged in activities like dining out and community events.


Freeda reflects on the most rewarding part of working with LIFTT: "Every person is a distinct individual and no two people with the same disability function in the same way. My role lets me use my skills to help our team serve our communities while ensuring we capture the data needed to continue funding.”

She emphasizes the complexity of LIFTT’s work, “People often think we have endless funds and can instantly provide ramps or showers. The reality is that we rely on grants, and those amazing things take time and effort. Most of us also live with disabilities, facing the same challenges as our consumers. We ask for grace and understanding as we do our best to make a difference.”

LIFTT’s dedication to helping individuals overcome barriers and achieve independence has made it a vital resource in Montana. Beyond the programs, LIFTT fosters a culture of inclusion through advocacy, peer mentorship, and community engagement.

As Freeda eloquently puts it, “It’s been said that LIFTT is one of the best-kept secrets in Southeast Montana—and that’s true. We strive to make a better community for everyone because helping one of us helps all of us.” Pullout quote?
